fix(subscriptions): token-scoped anchor claims and single root backfill master

- claimAnchor returns a per-claim token; releaseAnchorClaim only deletes the claim it owns, so an expired claimant can't free a newer one - reserve the root backfill per root ts, so losers woken together don't all post the first update into the thread - document why the claim TTL stays short
