JetstreamClient: primary failback and live idle-timeout master

- after a connection that delivered events drops, the next attempt starts at hosts[0] instead of sticking to fallback rotation until process restart - idle_timeout_ms fails over from a host that holds the socket open but goes silent, which TCP keepalive never fires on (replay_idle_timeout_ms still governs finite replay) Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>
