lexicons: promote media.origin to record + aac in segment audio master
place.stream.media.origin was a defs-style object — fine for embedding in a parent record but not publishable on its own. Promote main from type=object to type=record with key=any (lexicon doesn't have literal rkey syntax yet; convention is to use the blob's BDASL CID as the rkey, enforced at write time). Add a required `urls` array so the record can actually say *where* the blob is fetchable from — multiple entries leave room for HTTP plus future iroh/libp2p schemes. place.stream.segment#audio.codec gains "aac" alongside "opus" so VOD output (h264 + aac, produced by our gstreamer pipeline) can populate the same track-metadata shape that live segments already use.
