[docs] Make the root README an index and fix the credit format master

README.md is now an index, not a manual: one bullet per artifact — link plus one clause on what it does — with the per-item prose removed. Usage, configuration, troubleshooting, and credits belong to the artifact's own docs, which is why the deleted blocks reappear in the item READMEs and file headers rather than being lost. AGENTS.md: - Merge "README in lockstep" and "Docs" into a single "README" section; the two restated the same layering. Skills keep `skills/<name>/README.md` for human-facing setup and leave `SKILL.md` agent-facing only; everything else documents in its source header, and grows an item README only when there is more prose than a docstring should hold. - Add "Credits": one fixed sentence — "Verb from owner/project path (license) — delta", rendered as a Markdown link with a license and modification clause; plus Copied/Derived/Inspired verbs, a placement table, and the matching NOTICE entry shape. - Replace "Skill paths" with "Pi documentation": read pi's docs before extending pi, hosted at `pi.dev/docs/latest/<page>` and bundled under the install directory. The `{baseDir}` warning goes with it — no placeholder exists in this repo or upstream to guard against. Applied to the existing artifacts: goal, handoff, and notify credit in their header comments; the github skill credits in its README; kagi gains a `## Credits` section for the brave-search precedent it only carried in NOTICE. NOTICE entries now name `<owner>/<project>`, link the original file (handoff pointed at pi's docs site rather than its source), and all state what was modified. Root README's session-review entry links a README that does not exist yet.
