fix(cortex): restore resident start loop after brain refresh cutover master

_should_request_brain_refresh() had been inserted into the middle of Cortex.start(), leaving the spawn-worker thread start and the resident shutdown loop unreachable after a return. Cortex would connect to Callosum and immediately exit without draining talent requests. The regression test now covers Cortex.start() directly, which the existing tests did not.
