Handoff agent-chaining was built and tested but never wired into any agent workflow. Removes _spawn_handoff(), handoff_from parameter, handoff propagation in agent events, 5 handoff tests, and all documentation references. Observer "atomic handoff" (directory renames) and CALLOSUM "event-driven handoffs" (observer pipeline) are unrelated and left intact.
