Bump version 0.4.5 -> 0.4.6 and cut the 0.4.6 CHANGELOG block (backlog visibility + reprocess affordances, unified on-device Local provider with macOS cogitate, linux transcription out of the box, local journal commands moved to the journal surface, pending-count fix).
