keep an ack after the journal accepts a segment master

The app records an ack when the journal durably accepts a segment, and it does not ask the journal again on later passes while that ack still matches. Segments the journal rejects stay in place. A segment that was already on disk at the first start of this version, or whose ack belongs to another pairing, is checked once with a segments listing. The root manifest and per-day manifest reads are gone.
