move goals to the ipython goal skill (#159) master

* move goals from harness tools to the ipython goal skill The model's only built-in tool is now ipython. The rlm.run comm channel is generalized into a typed host bridge (comm target host.request, dispatched via hostHandlers), rlm.host_request() is public for kernel-side skills, and goals ship as the bundled python skill 'goal' (goal.get / goal.create / goal.complete) backed by AgentSession state. Goal re-prompting, budgets, and goal_update events are unchanged. * let goal.create replace terminal goals and explain pending-goal rejections A complete or error goal is a finished record, not a pending objective, so goal.create now starts a fresh goal over it (matching what /goal already allowed for users). Active, paused, and budget-limited goals still reject creation, each with a message telling the model what to do instead. * harden the goal host bridge against review-bot findings Completing a goal now drops any stale budget-limit steer queued earlier in the same turn, host replies with an unexpected status reject instead of hanging the kernel future, and a payload type key can no longer reroute a host request. * fix(coding-agent): stop goal completion usage overcount
