docs: shoot the * badges, and the ! that is now only a failure master

The capture script's tile list gains the three `*` states; the old `badge-warning.png` becomes `badge-failed.png`, which is the same amber `!` with the narrower meaning. status-states.md and the README describe the modifier, and the badge table gains a row per `*` state.
