docs(policy): say what a declaration's revision number is master

`set::declarations` stamps every declaration revision 0, so the two numbers a `LastGoodFallback` reports are a label rather than an ordering; the field says so and points at where the fallback actually keys. The schema crate's own doc now describes the methods it validates, which `didbot`'s wire conformance tests already exercise.
