feat(dashboard): add revoke-tokens, and an entry per account to end-logins master

POST /dashboard/api/accounts/{did}/revoke-tokens ends every token that authenticates as the account and answers with its tokensRevoked entry. end-logins answers with the loginsEnded entry each account it reached gained.
