docs(infra-review): close the two findings that held up an apply master

`--record-target` carries the instance's Elastic IP, Terraform owns the apex address record, `ensure_server_account` owns the apex account, and `shutdown_signal` handles SIGTERM. CLAUDE.md pointed agents at a TODO.md; the epic's own `## Done` list is where an item gets checked off.
