feat(agentd): say a narrowed approval covers the whole request master

A narrowed `didbot-oauth` line now carries `approves=asked ceiling-checked=each-use`, and an approval prints what the ceiling grants as `granted now:`. The ceiling is asked again at every use, so a cut is withheld for now rather than refused.
