docs(oauth): redraw the sign-in as the approve path master

The sequence still showed a headless browser reading a one-time reference off the authorize page and a hook stamping the acting DID onto a confirm call. None of that exists: the request becomes a decision record, the daemon polls for it, and the agent answers with a token that names no account.
