fix(slack): bound the permalink lookup; address source_ref review master

- permalinkFor races chat.getPermalink against a 3s timeout so a slow Slack API no longer delays the agent turn; a timeout logs and returns undefined like a failed lookup - comment why appendIncidentEvent uses an explicit-undefined check for sourceRef instead of ?? - backfill script logs progress every 100 rows on --apply
