slack: channel context, incident follow-ups, welcome actions master

- mentions see the channel before them (with author and UTC time), Slack message links are read with a membership check, and a thread remembers what its approved cards created - declaring offers "Link this channel instead" in the source channel and a drafted status report card in the incident channel - the status report form gets Started at, which moves the incident's start back when earlier - approved cards show the next step with buttons - the welcome card offers Catch up, Declare incident, Publish status update
