feat: incident api master

Expose managed incidents on the ConnectRPC API as openstatus.incident.v1.IncidentService (13 RPCs: declare, get, list, update, set status, note, link/unlink status report, close, delete, get/update/approve postmortem). - services: listIncidents gains a closed filter and total count; getIncident returns allowedTransitions/deletable; status report reads return incidentId; getPostmortem loads the approver. - services: incident/effects.ts holds the after-verb side effects (commander email, Slack channel open/announce/archive) shared by tRPC, Slack and RPC; resolveDashboardUrl replaces four copies. - rpc: ConflictError now maps to FailedPrecondition for every service. - proto: incident/v1, plus incident_id on status reports. - docs: Incident Service SDK page, reference enums, changelog.
