feat(ai-commit): implement heuristic-based auto-refinement for poorly formatted or overly long commit messages master

Add detection thresholds for body character count (800) and line count (8), enforce structural validation, and introduce optional recent commits context to improve refinement accuracy while preventing infinite loops
