Add Last.fm integration: read-only source + match disambiguation master

Last.fm predates PKCE, so auth uses its own web-auth token flow (MD5-signed auth.getSession exchange) instead. The adapter is read-only by design — usable as a migration source (loved tracks) but never a destination — and track.getInfo backs a disambiguation hook the matching engine can call for medium-confidence fuzzy matches.
