Add `// Source:` headers to track upstream repositories for each extension, making it easy to identify where they came from and manage updates. Include a new `scripts/pi-extensions-sync` bash utility for pulling upstream changes and diffing extensions against their sources. Remove the large review extension (2441 lines) as it's being consolidated elsewhere. Add SOURCE.md for intercepted-commands documentation.
