Move to Opus 5 and keep input history with the player master
Opus 4.7 is a generation behind, and Opus 5 costs the same at $5 and $25 per million tokens, so this is a straight swap at every call site: DM engine, planner, seeder, arc planner, ticker, and advancement evaluator. The utility model drops its date suffix to the `claude-haiku-4-5` alias, which is still the newest Haiku. Effort levels are unchanged and have not been re-tuned for Opus 5. Command history moves from `~/.storied_history` to `~/.storied/players/{player_id}/input-history`, because it is player data and belongs beside the character and the notes. Setup now runs after `configure()`, so a `--base-path` or `--sandbox` run writes history where the rest of that run's data lives. The old path also broke startup here: this venv's readline is backed by libedit, which rejects a history file GNU readline wrote, and that OSError escaped `cmd_play`.
