feat: adaptive layout, quit confirmation, step navigation, ctrl+p palette master

- layout adapts to terminal height via useTerminalSize hook - ThinkingPane/AgentSteps/ResultPane constrained to terminal bounds - Ctrl+P opens command palette (replaces :) - Escape shows quit confirmation, second escape quits - Ctrl+←/→ navigates steps in timeline
