`[[Note#Heading]]` rendered as `NoteHeading`. The `#` is a `WikiLinkMark`, and the decoration hid every mark alongside the brackets — so the note name and the heading ran together with nothing between them. The `#` now stays visible and stays inside the link-styled span, giving `Note#Heading`. A same-document `[[#Heading]]` renders as `#Heading` for the same reason: the marker is what signals it points at a heading rather than a note.
