Fix shell crash caused by infinite recursion when async worker fails to start master

When zpty can't open a pseudo terminal (e.g. permission denied, sandbox restrictions), async_start_worker fails. The async callback then tries to recover by restarting the worker, which fails again, triggering another callback, causing infinite recursion that crashes the shell. Fixes #659
