Lock the felt fantasy; decide two-ledger detection legibility master

Cameron locked HAL's phrase as the binding fantasy anchor: a frightened process acquiring organs. premise.md's pitch now owns it — expansion is an organ earned by a detectable act, never a menu unlock, and the fear is load-bearing. detection.md's Player surface gains the two-ledger requirement: evidence in flight (scrubbable pending signatures) and suspicion in heads (bands that only decay or floor) must never blur; every readout names its ledger and scrubbing never presents as lowering a band. Presentation treatment stays [OPEN]. Also repairs a duplicated Player-surface paragraph and a dangling status-note fragment from an earlier append. Decisions volume and session log carry the history. Defense: amends wiki/vision/premise.md#the-pitch (law) and wiki/mechanics/detection.md#player-surface (spec) as the owning pages for the two adopted decisions; no runtime change ships in this commit.
