Generate DEVLOG and specs board from unique sources. master

tools/ledger_index.sh rebuilds wiki/log/DEVLOG.md from Type:log session files and wiki/process/specs.md tables from each Type:spec Status/Stage. check.sh docs path fails if indexes are stale. Agents add a session log and re-run the generator instead of hand-merging ledger tips. Defense: Implements wiki/process/agent-scale.md slice C (append-only ledgers and generated indexes).
