feat: handle missing frontmatter in parseFrontmatter and updateFrontmatterWithAtUri master

Add unit tests for markdown.ts and make frontmatter optional: parseFrontmatter now extracts title from headings when no frontmatter is present, and updateFrontmatterWithAtUri creates a new frontmatter block when none exists.
