menuband: chooser contrast + layout renames + keymaps-paper button master

- InstrumentMapView: MIDI-OUT / SAMPLE / radio cells get visible filled chips when inactive (0.10 → 0.30 fill, brighter stroke, full-alpha labels) instead of the barely-there outline — readable in light + dark. - Expanded keymap layout toggle renamed: Notepat → Default, Conventional → AWSED. - Added a '?' button in the mode row that opens the keymaps paper (bundled PDF in Preview, else the canonical hosted URL).
