v1.1.1 — fix blank window on launch

The v1.1.0 CSP middleware stamped script-src 'none' on every shell response, including the compiled JS bundle and stylesheet that index.html fetches with crossorigin. With no Access-Control-Allow-Origin on those responses, the header failed WebKit's same-origin CORS check: both assets were blocked and the app rendered nothing on launch.
